Laying inset manhole cover - How do i install one

hello to everyone, this is my first thread and can I say what a great site this is.
I need some advice on how to install a inset manhole cover. I have seen the advice here on the site however the problem I have is the existing manhole is now 400mm below the level of the patio and so the tray would be below this level if I put concrete around the existing manhole and sit the tray on top of it?
So whats the best way to get around this problem??

You will need to build it up with engineering bricks. See here for how it's done
