when cutting in, do you treat the base line to the pavement (where it meets the pavement) the same as the other three sides, regarding each block should have three uncut edges and no cut should be less than half a block.

Yes - the rules for cutting-in apply everywhere - to all edges, around manholes or gullies, around other pavement features, everywhere. :)
